Membership

Fellow

After three years as an Associate, you are eligible to become a Fellow of the College. In order to attain this change in status, you must submit:

  • A completed Fellow application (including release form)
    Associate to Fellow Member Application:   Word  |  PDF

  • A completed case log form with 300 cases completed after your training - ACMS Case Log:   Excel  |  PDF

The application deadline is August 1.
